Chapter One
This is my sample book to see if I formatted everything correctly for Smashwords. Mostly I created it for myself to ensure that I can create a readable book using the Smashwords format. You are welcome to read it too, if you wish. First create your Table of Contents with each item that you wish to appear in the list. Then list your chapter headings with a pilcrow separating each chapter heading and one pilcrow below the last chapter heading.
Chapter Two
Then save as a new file and hyperlink all the chapter headings to 'ref_TOC' you will get an error saying that 'ref_TOC' does not exist but proceed anyway.
Chapter Three
Then hyperlink the 'Table of Contents' title to a dummy link, and then create a bookmark for 'Table of Contents' which should now accept 'ref_TOC'. Test out all the chapter headings and ensure that they all link back up to the Table of Contents. Delete the hyperlink on 'Table of Contents'.

Chapter Four
Finally, create bookmarks for all the chapter headings, which are all hyperlinked to the TOC. Lastly, hyperlink all the Table of Content items to the respective chapter headings. Done!
